Send a referral
Call the front desk or fax the referral to (704) 555-0108. We confirm that we have it and call the family the same working day.
What helps us
The child's name, a parent or guardian's phone, your contact details, the area of concern and any precautions you want us to follow.
What you get back
A written note after the first visit and when the plan of care changes, sent the way you prefer.
Families who prefer Spanish
Tell the family to ask for Mayela Leon, Mariluz Ibanez or Maximiliano Romero, or tell us and we schedule them with one of them.
